What PHQ-9 measures

The nine-item symptom module of the Patient Health Questionnaire covers interest, mood, sleep, energy, appetite, self-worth, concentration, movement and thoughts of death or self-harm. Developed by Kroenke, Spitzer and Williams; validated in 2001.

How scoring works

Sum nine responses scored 0–3: total 0–27, without reverse items. Symptom ranges: 0–4 minimal, 5–9 mild, 10–14 moderate, 15–19 moderately severe, 20–27 severe. No diagnostic algorithm is applied.

About PHQ-9

Does a high score diagnose depression?

No. A professional assessment considers functioning, medical causes, other symptoms and your circumstances.

Does a low score rule out danger?

No. Thoughts of death or self-harm deserve attention regardless of the total. This questionnaire cannot determine immediate risk.
